2025 Award Nomination Form
The Awards Committee requests that letters of support highlight specific achievements and contributions of the nominee that warrant recognition by SMDM. We are committed to inclusivity and aim to reflect the diversity of our field in our recognitions.
Each nomination will be reviewed by the Awards Committee to ensure they meet the criteria for the respective awards. All nominees will be notified of the decisions following the SMDM Board's approval. Award recipients will be honored at the 2025 SMDM Annual Meeting in June.

2025 SMDM AWARDS
Career Achievement Award
The Career Achievement Award recognizes senior investigators who have contributed significantly to the field of medical decision making. Nominees need not be members of SMDM.

How does the nominee’s career demonstrate leadership and impact in the field of medical decision making?
SMDM Award for Distinguished Service
This award recognizes service to SMDM in terms of leadership, role in the operations of the Society, and contributions to the scientific and educational activities of the Society. The nominee must be a member of SMDM.
Questions to stimulate your thinking about worthy nominees:

Who comes to mind as a person to call on for help if you need to get something accomplished for SMDM?

Who has gone above and beyond for SMDM?

Who demonstrates efforts to promote a welcoming atmosphere for SMDM?

Who do you think demonstrates efforts to promote a welcoming atmosphere or fosters a diverse, inclusive global research culture in our society?
John M. Eisenberg Award for Practical Application of Medical Decision Making Research
This award recognizes sustained leadership in translating medical decision-making research into practice, including taking exceptional steps to communicate the principles and/or substantive findings of medical decision-making research to policy makers, or clinical decision makers, or the general public. The nominee need not be a member of SMDM.
Questions to stimulate your thinking about worthy nominees:

When you come across a good idea in your work or practice, embodying solid medical decision-making principles, who has worked to bring that idea into practice?

Who, for example, successfully informed medical guideline development or implementation of innovative technologies in health care or public health?
-
This award also addresses mid-career individuals who have demonstrated the impact of their work, for example, impactful diversity and equity research.
Outstanding Paper by a Young Investigator
The award was conceived as a means of recognizing outstanding work and assisting the recipient in the promotion process.

The nominee must be the first author of the paper.
-
The nominator must be a member of SMDM.

The paper nominated must appear in a peer-reviewed journal with a publication date of 2023. Online journals qualify, provided they are peer-reviewed.*

Papers will be judged for their importance and originality.
-
The nomination must include a copy of the article preferably in pdf format, including complete journal citation. A letter of support explaining the importance of the paper is required.
*This award is limited to a paper published in the calendar year prior to the award (journal must be dated in 2023 for the 2024 award). The nominee must be in the first six (6) years of full-time employment after the end of “training”; however, that is defined within the country and field of the nominee.

The SMDM Young Scholars Award in Health Policy
Presented in honor of Sandy Schwartz
This award is given in honor of Sandy Schwartz for his commitment to mentorship in the field of health policy and his support of young scholars in the Society for Medical Decision Making. The SMDM Young Scholars Award recognizes a promising Scholar who is new to or early in their career in the field of Medical Decision Making but has the potential for impact and leadership in the field and in the Society of Medical Decision Making. Through this award we aim to support young scholars with diverse backgrounds, experiences, and scientific disciplines to become future leaders in SMDM. The nominee must be a member of SMDM.
The SMDM International Young Scholars Award
Presented in honor of Murray Krahn
This award is given in honor of Murray Krahn for his commitment to mentorship and international collaboration in the field of Health Technology Assessment and his support of young international scholars in the Society for Medical Decision Making. The SMDM Young Scholars Award recognizes a promising Scholar who is new to or early in their career in the field of Medical Decision Making but has the potential for impact and leadership in the field and in the Society of Medical Decision Making. Through this award we aim to support young scholars with diverse backgrounds, experiences, and scientific disciplines to become future leaders in SMDM. The nominee must be a member of SMDM. Preference is given to nominees who engage in international collaboration and/or reside in LMIC (low- and middle-income countries).
